Indicator library
Humanitarian M&E indicator library
A free, searchable reference of standard humanitarian monitoring and evaluation indicators — with definitions, disaggregation, means of verification and results level. Adapt them for your logframe, proposal or donor report.
Browse by sector and results level, copy what fits, and adapt it to your context. Each indicator is a curated reference aligned with recognised humanitarian frameworks — not an official registry entry, so always confirm the exact wording your donor requires.
Last reviewed: September 2026
How this library is built & reviewed
Each indicator is compiled by humanitarian M&E specialists and passed through an independent review for correct results level, sound numerator and denominator, realistic means of verification, and appropriate disaggregation.
Where an indicator aligns with a recognised framework — Sphere, the sector clusters, IASC, CHS, INEE, IPC, WHO and others — we link its official source so you can verify it directly. Indicators that are common practice with no single official source are labelled as such.
The library concentrates at outcome and output level, where indicators are comparable across projects. Impact indicators are few (they are measured at population level and hard to attribute), and activity indicators are deliberately sparse — activities are project-specific and are normally tracked as workplan milestones rather than standard indicators.
This is a reference to adapt, not an official registry. Baselines and targets are project-specific; always confirm the exact definition your donor or cluster requires before reporting.

Cash & Voucher (CVA)
Outcome% of monitored markets assessed as functional and adequately supplied with essential MEB commoditiesCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Number of monitored markets meeting the defined functionality criteria (essential MEB commodities available, prices stable/non-volatile, and physically accessible) during the monitoring round
Denominator: Total number of markets monitored in the same reporting round
Assess each monitored market against predefined functionality criteria (commodity availability, price stability, and access for MEB items) and calculate the share meeting the threshold.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Market functionality and price monitoring surveys, trader interviews, and rapid market assessments
- Guidance & common pitfalls
- A cash-appropriateness/context monitoring measure rather than a result the programme produces on its own; deteriorating market functionality or sharp price rises may signal a need to revisit the modality or transfer value. Define functionality thresholds and track the same markets over time for comparability.

Outcome% of MPCA recipient households able to meet their priority basic needs after assistanceCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Number of surveyed MPCA recipient households reporting they were able to cover their self-identified priority basic needs (benchmarked against the Minimum Expenditure Basket) after receiving assistance
Denominator: Total number of MPCA recipient households surveyed
Assess household ability to meet priority needs relative to the Minimum Expenditure Basket via expenditure recall and self-report during PDM, expressed as a percentage of households surveyed.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Post-distribution monitoring (PDM), household survey, expenditure/MEB analysis
- Guidance & common pitfalls
- Anchor to a locally defined and periodically updated MEB; distinguish partial from full coverage of needs and interpret alongside transfer value adequacy. Self-report is subject to recall and courtesy bias.

Outcome% of MPCA recipient households with an acceptable Food Consumption ScoreCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Number of surveyed MPCA recipient households whose Food Consumption Score falls in the 'acceptable' category
Denominator: Total number of MPCA recipient households surveyed
Compute the Food Consumption Score from 7-day food group frequency weighted by nutritional value, classify against standard thresholds, and report the share in the acceptable band.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Household survey, post-distribution monitoring, baseline/endline comparison
- Guidance & common pitfalls
- Apply context-appropriate thresholds (standard vs. high-sugar/oil consumption) consistently across rounds; FCS reflects dietary diversity and frequency, not intra-household distribution or micronutrient adequacy.

Outcome% of MPCA recipients reporting they could decide independently how to use the assistanceCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Number of surveyed MPCA recipients reporting they were able to decide freely and independently how the cash was spent
Denominator: Total number of MPCA recipients surveyed
Self-reported measure from PDM asking whether the recipient controlled spending decisions, expressed as a percentage of respondents.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Post-distribution monitoring, household survey, key informant interviews
- Guidance & common pitfalls
- Captures the dignity and flexibility rationale for unconditional cash; probe for household-level and gendered control dynamics rather than assuming the registered recipient decides.

Outcome% of MPCA recipients satisfied with the cash transfer value and with the delivery processCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Number of surveyed MPCA recipients who report being satisfied or very satisfied with the transfer value AND, separately, with the delivery/collection process
Denominator: Total number of MPCA recipients surveyed
Collect satisfaction on a standard scale for transfer adequacy and for delivery experience as two separate items; report each sub-component and, where a combined figure is used, count recipients satisfied on both.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Post-distribution monitoring, recipient satisfaction survey
- Guidance & common pitfalls
- Report value and process satisfaction separately as the primary figures — a single combined score is double-barrelled and can mask problems with cash-out points, fees, or distance for specific groups. Watch for courtesy bias.

- Definition & calculation
Numerator: Number of surveyed MPCA recipients reporting they felt safe travelling to, collecting, and holding/using the cash assistance
Denominator: Total number of MPCA recipients surveyed
Self-reported safety perception collected during PDM covering travel to collection points, the collection event, and holding/using cash, expressed as a percentage.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Post-distribution monitoring, protection monitoring, key informant interviews

OutcomeAverage reduced Coping Strategies Index (rCSI) score among MPCA recipient householdsCash & Voucher (CVA)Score / index
- Definition & calculation
Calculate the standard reduced Coping Strategies Index from the frequency and severity weighting of the five consumption-based coping behaviours over the past 7 days, averaged across surveyed recipient households.
- Means of verification
- Household survey, post-distribution monitoring, baseline/endline comparison
- Guidance & common pitfalls
- A lower score indicates fewer/less severe negative coping; compare against baseline and a non-recipient reference where feasible, as rCSI is sensitive to seasonality and shocks. Reported as an average score — no numerator/denominator.

Output% of targeted households reached with multi-purpose cash assistanceCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Number of targeted households that received at least one planned multi-purpose cash transfer during the reporting period
Denominator: Total number of households in the approved MPCA target/caseload for the same period
Divide households that actually received a transfer by the total planned target caseload and multiply by 100.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Distribution records reconciled against the targeting/registration list
- Guidance & common pitfalls
- Measures targeting delivery, not adequacy; investigate large gaps between reached and targeted (access constraints, exclusion errors, or over-optimistic targets).

Output% of the Minimum Expenditure Basket (MEB) covered by the multi-purpose cash transfer valueCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Value of the multi-purpose cash transfer delivered per household per month
Denominator: Cost of the locally defined Minimum Expenditure Basket (MEB) for an equivalent household size and period
Divide the per-household monthly transfer value by the current MEB cost for a comparable household size and multiply by 100, updating the MEB with prevailing market prices.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Transfer value/programme design records, current MEB costing from the Cash Working Group, and market price monitoring
- Guidance & common pitfalls
- Measures the adequacy of the transfer design against the MEB, not what households actually achieved; adjust for household size and refresh the MEB as prices change so the ratio is not overstated by an outdated basket.

OutputNumber of individuals reached with multi-purpose cash assistanceCash & Voucher (CVA)Number
- Definition & calculation
Count of unique individuals in assisted households who received at least one multi-purpose cash transfer during the reporting period, de-duplicated across cycles and modalities.
- Means of verification
- Distribution records, beneficiary management system (token/e-voucher/bank platform), de-duplicated registration database
- Guidance & common pitfalls

Activity% of multi-purpose cash transfers disbursed within the planned distribution timeframeCash & Voucher (CVA)Percentage
- Definition & calculation
Numerator: Number of scheduled MPCA transfers that reached recipients on or before the planned disbursement date (or within the agreed tolerance window)
Denominator: Total number of MPCA transfers scheduled for disbursement in the reporting period
Compare actual disbursement/collection dates against the planned schedule and calculate the share meeting the agreed timeframe.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Financial service provider payment logs, distribution schedule, disbursement reconciliation reports
- Guidance & common pitfalls
- Define the acceptable timeframe and cycle frequency in advance; recurring delays undermine households' ability to plan and can force negative coping.

CCCM
Outcome% of displacement sites with a functional, accessible complaints and feedback mechanismCCCMPercentage
- Definition & calculation
Numerator: Number of assessed sites with an operational complaints and feedback mechanism that receives, refers and responds to feedback within the agreed timeframes
Denominator: Total number of assessed displacement sites
Sites are assessed for the presence and functioning of a feedback channel (multiple accessible modalities, logging, referral, and response tracking); functional sites are divided by all assessed sites.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- CFM logs; site monitoring; accountability audit
- Guidance & common pitfalls
- A box or hotline alone is not enough; assess whether feedback is acted on and whether channels are accessible to women, children and persons with disabilities.

Outcome% of displacement sites with a functioning camp management or coordination structureCCCMPercentage
- Definition & calculation
Numerator: Number of assessed sites with an active management/coordination structure that meets regularly and performs the agreed core coordination functions
Denominator: Total number of assessed displacement sites
Sites are assessed against agreed criteria for a functioning structure (regular meetings, service coordination, information management); qualifying sites are divided by all assessed sites.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Site monitoring reports; coordination meeting minutes; site management assessment
- Guidance & common pitfalls
- 'Functioning' must be defined by activity, not mere existence on paper; a named committee that never meets should not count. Distinguish from the count of sites where a structure has simply been established.

Outcome% of monitored displacement sites with no outstanding critical (life-threatening) service gap at the end of the reporting periodCCCMPercentage
- Definition & calculation
Numerator: Number of monitored displacement sites with zero unresolved critical (life-threatening) service gaps recorded at reporting period end
Denominator: Total number of displacement sites monitored during the reporting period
Using site monitoring and service-mapping data, classify each site by whether any critical service gap remains open at period end; divide sites with none by all monitored sites and multiply by 100.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Site monitoring reports and service-mapping / gap-tracking database
- Guidance & common pitfalls
- Critical gaps are those threatening life or basic survival (e.g. no safe water, no access to health care, no functioning latrines); agree the threshold with the coordination group. This captures the end-state coverage condition, distinct from indicators measuring referral or resolution speed.

- Definition & calculation
Numerator: Number of identified service gaps referred to responsible actors and resolved or actioned within the agreed timeframe
Denominator: Total number of service gaps identified through site monitoring during the reporting period
Gaps logged during site monitoring are tracked through referral to resolution; those closed or actioned within the agreed timeframe are divided by all gaps identified.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Service monitoring reports; referral tracking log; coordination minutes
- Guidance & common pitfalls
- Measures the effectiveness of CCCM's gap-identification and advocacy role; requires a functioning referral and tracking system and clearly defined timeframes. Note that resolution often depends on other actors beyond the reporting agency's control.

- Definition & calculation
Numerator: Number of assessed sites meeting the agreed minimum usable surface area per person and the safe site-planning layout criteria
Denominator: Total number of assessed displacement sites
Site area, population and layout are measured and compared against the agreed planning standard (e.g. minimum usable m² per person, firebreaks, drainage); compliant sites are divided by all assessed sites.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Site profiling; GIS/site mapping; camp management technical assessment
- Guidance & common pitfalls
- Overcrowding drives protection and public-health risk; measure usable area (excluding unusable terrain) rather than gross site area, and note that compliance is influenced by population movement beyond the intervention's control.

Output% of affected people who can identify at least one available feedback or complaint channelCross-cutting / MEALPercentage
- Definition & calculation
Numerator: Number of surveyed affected people who can correctly identify at least one available channel to provide feedback or make a complaint
Denominator: Total number of affected people surveyed
Ask respondents to name an available feedback/complaint channel and count those who identify at least one correctly, divided by all respondents.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Household survey, post-distribution monitoring, community perception survey

Output% of monitored service and distribution points meeting protection mainstreaming standardsCross-cutting / MEALPercentage
- Definition & calculation
Numerator: Number of monitored service delivery or distribution points assessed as meeting all agreed protection mainstreaming criteria (safety and dignity, meaningful access without barriers or discrimination, and accountability/participation)
Denominator: Total number of service delivery or distribution points monitored during the reporting period
Each monitored point is assessed against a standardised protection mainstreaming checklist, and points meeting all core criteria are divided by all points monitored.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Protection mainstreaming monitoring checklist; site monitoring reports; post-distribution monitoring
- Guidance & common pitfalls
- Assesses how assistance is delivered (safely, with dignity, and equitably) rather than whether it was received, and criteria should cover access for at-risk groups. As a checklist-based measure of delivery conditions it is an output-level quality measure, not a substitute for specialised protection programming or affected-people self-reports of dignity.

Denominator: Total number of program participants surveyed
Combine documented consultation activities with participant self-report of having been meaningfully consulted, expressed as a percentage of respondents.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Consultation and community meeting records, participant survey, community perception survey

Track eligible supported children over time and calculate the percentage who enrol in and are retained by the formal/national system at the follow-up point.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Formal school enrolment records, certification/examination records, longitudinal tracking of learners

Denominator: Estimated total number of school-aged children in the target population, derived from population data or a needs assessment
Divide the number of target children enrolled in and regularly attending supported learning environments by the estimated total number of school-aged children in the target area, then multiply by 100.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Enrolment/attendance registers cross-checked against population estimates; household survey
- Guidance & common pitfalls
- Headline access/coverage measure for EiE. Denominator: Total number of supported schools assessed
Each supported school is assessed against agreed WASH-in-schools standards (safe water, adequate sex-segregated latrines, functional handwashing), and schools meeting all criteria are divided by all schools assessed.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- School WASH facility assessment/checklist; monitoring visit reports
- Guidance & common pitfalls

Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Household food security survey (HHS module)
- Guidance & common pitfalls
- HHS captures severe household food deprivation and is validated for cross-cultural comparison; it is insensitive to milder food insecurity, so pair it with FCS or rCSI for programme monitoring. OutcomeMean reduced Coping Strategies Index (rCSI) among target householdsFood SecurityScore / index
- Definition & calculation
Sum the frequency of the five standard food-based coping behaviours over the past 7 days, each multiplied by its universal severity weight, and report the sample mean.
- Means of verification
- Household food security survey (rCSI module), post-distribution monitoring
- Guidance & common pitfalls
- A falling rCSI indicates improved food access (lower is better); the reduced (5-strategy) version is standardised for cross-context comparison, so avoid substituting context-specific strategies where comparability is required. Aligned with Food Security Cluster and IPC conventions.

Output% of assisted households that received their full intended entitlement (correct quantity/value and quality)Food SecurityPercentage
- Definition & calculation
Numerator: Number of sampled assisted households confirming at post-distribution monitoring that they received the complete ration or transfer value in the correct commodity mix and quality
Denominator: Total number of assisted households sampled at post-distribution monitoring
Divide the number of sampled households confirming receipt of the full, correct entitlement by the total sampled, expressed as a percentage.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Post-distribution monitoring (PDM) survey, distribution reconciliation records
- Guidance & common pitfalls
- Measures delivery accuracy/quality, not adequacy of the ration itself; triangulate self-reported receipt with distribution reconciliation records and define 'full entitlement' against the planned ration or transfer value in advance. Aligned with Sphere and CHS quality standards.

- Definition & calculation
Count households that received at least one qualifying agricultural input package (e.g. seeds, tools, fertiliser, livestock) or completed a defined technical training, deduplicated across activities so a household receiving both is counted once.
- Means of verification
- Input distribution records, training attendance sheets, beneficiary registers
- Guidance & common pitfalls
- Disaggregate by sex of household head where household-level counts are used; specify the input package so quantity/quality can be assessed separately, and do not double-count a household under both inputs and training. Aligned with Food Security Cluster and Sphere output conventions.

- Means of verification
- Distribution records, beneficiary registration database, transfer/redemption reports

ImpactCrude mortality rate (CMR) in the affected population (deaths per 10,000 persons per day)HealthRatio
- Definition & calculation
Divide the number of deaths in the population during the recall period by the mid-period population and by the number of days, expressed per 10,000 per day.
- Means of verification
- Mortality surveillance; community death registers; retrospective mortality survey
- Guidance & common pitfalls

- Means of verification
- Mortality surveillance; community death registers; retrospective mortality survey
- Guidance & common pitfalls

Numerator: Number of surveyed caregivers of children under 5 who correctly name at least two recognized danger signs (e.g. inability to drink/breastfeed, convulsions, lethargy, fast/difficult breathing)
Denominator: Total number of caregivers of children under 5 surveyed
Ask an unprompted (open-recall) question and score responses against the standard danger-sign list; report the recall threshold used.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- KAP survey; household survey

Denominator: Estimated total number of children aged 6-59 months in the target area
Divide doses administered to the eligible age cohort by the estimated eligible population; validate with a post-campaign coverage survey where feasible.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Vaccination tally sheets and registers; post-campaign coverage survey; administrative denominators
- Guidance & common pitfalls
- Administrative coverage can exceed 100% due to weak denominators or in-migration, so triangulate with survey data. In acute emergencies measles is a priority antigen from 6 months of age.

Denominator: Total number of deliveries (or expected deliveries) in the target population during the period
Divide births attended by a defined skilled provider by total (or expected) deliveries; expected births can be estimated by applying the crude birth rate to the population.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Facility maternity registers; household survey; community birth records
- Guidance & common pitfalls

Denominator: Total number of MHPSS clients with both a baseline and a follow-up assessment in the period
Compare each client's baseline and follow-up scores on a validated, context-adapted instrument and count those crossing the defined improvement threshold.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Validated, context-adapted pre/post assessment tools (distress or functioning scales) recorded in case files
- Guidance & common pitfalls

Denominator: Total number of pregnant women (or expected pregnancies) in the target population during the period
Divide women recording four or more ANC contacts by the total pregnant/expected-pregnant population; align to the WHO 8-contact model where adopted, but always report the threshold used.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- ANC registers; home-based maternal records; household survey

Denominator: Total number of clients surveyed on service satisfaction during the period
Survey a representative sample of service users, ideally away from the provider, and calculate the proportion meeting the positive-satisfaction threshold.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Exit interviews; client satisfaction survey; feedback mechanism data

Denominator: Total number of women of reproductive age (15-49) in the same reference group surveyed
From survey data, divide women reporting current use of a modern method by all women of reproductive age in the defined reference population.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Household / KAP survey; family planning service registers
- Guidance & common pitfalls
- Specify the reference population (all women vs. married/in-union vs. sexually active), as it strongly affects the value. Service-register 'new acceptors' is an activity proxy, not the prevalence rate.

OutcomeNew outpatient department (OPD) consultation utilization rate (new consultations per person per year)HealthRatio
- Definition & calculation
Divide the total number of new (first-visit) outpatient curative consultations in the period by the estimated catchment population, then annualize (multiply a monthly figure by 12/number of months in the period).
- Means of verification
- Facility OPD registers / HMIS; catchment population estimates
- Guidance & common pitfalls
- Count only new consultations for a new illness episode, not repeat/follow-up visits, to avoid inflation. The Sphere reference range is roughly 1-4 new consultations per person per year in stable emergencies; both very low and very high values can signal access or quality problems. Nutrition
ImpactPrevalence of global acute malnutrition (GAM) among children aged 6-59 monthsNutritionPercentage
- Definition & calculation
Numerator: Number of children aged 6-59 months with weight-for-height z-score < -2 and/or bilateral pitting oedema in the surveyed sample
Denominator: Total number of children aged 6-59 months measured in the representative survey sample
Measure weight-for-height and check for oedema in a probability sample of children 6-59 months, then estimate the proportion classified as acutely malnourished using WHO Child Growth Standards.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- SMART nutrition survey or equivalent population-representative anthropometric survey
- Guidance & common pitfalls
- A population-level status indicator best measured through a representative survey (e.g. SMART), not programme records; attribution to a single project is weak, so use it to describe context/impact severity alongside IPC acute malnutrition classification rather than as a direct project output.

Denominator: Total number of caregivers of children in the treatment programme surveyed
Through exit interviews or a periodic survey, calculate the proportion of caregivers rating overall service satisfaction positively on a defined scale, triangulated with feedback mechanism data.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Exit interviews, beneficiary satisfaction survey, and complaints and feedback mechanism data

Denominator: Total number of caregivers of children under 2 surveyed
Ask caregivers structured questions on recommended IYCF practices and calculate the proportion answering at least two correctly against a predefined scoring key.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- KAP survey or post-counselling knowledge assessment against a predefined answer key

Outcome% of children aged 6-23 months who received a minimum acceptable diet (MAD) in the previous 24 hoursNutritionPercentage
- Definition & calculation
Numerator: Number of surveyed children aged 6-23 months who met both minimum dietary diversity and minimum meal frequency (and, for non-breastfed children, the milk feed criterion) in the previous day
Denominator: Total number of children aged 6-23 months surveyed
Combine minimum dietary diversity and minimum meal frequency (with milk feeds for non-breastfed children) from 24-hour recall to derive the composite MAD indicator per WHO/UNICEF definitions.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Household or KAP survey using the standard WHO/UNICEF IYCF recall module
- Guidance & common pitfalls
- MAD is a composite indicator that depends on accurate breastfeeding status classification; report breastfed and non-breastfed children separately as the component criteria differ, and do not interpret it as a nutrient-adequacy measure.

Denominator: Total number of children with MAM who exited the supplementary feeding programme during the reporting period (recovered + died + defaulted + non-recovered)
Divide children discharged as recovered from MAM treatment by all MAM exits (recovered, died, defaulted, non-recovered) in the same reporting period, expressed as a percentage.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Supplementary feeding programme (SFP/TSFP) registers and IMAM/CMAM monitoring database
- Guidance & common pitfalls

Denominator: Total number of children with SAM who exited the programme during the reporting period (cured + died + defaulted + non-recovered)
Divide the number of defaulters by the total number of exits in the same reporting period, expressed as a percentage.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- OTP/SC registers, defaulter tracing records and CMAM/IMAM database
- Guidance & common pitfalls
- A defaulter rate above the Sphere alarm threshold (>15%) signals access, quality or acceptance problems. Apply a consistent absence definition and reconcile with defaulter tracing so that unreported deaths are not misclassified as defaults.

Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- OTP/SC discharge registers and CMAM/IMAM monitoring database
- Guidance & common pitfalls
- Report as part of the standard performance basket (cure, death, defaulter and non-recovery rates), which should sum to 100%; the Sphere reference for cure is >75%. Exclude transfers to other treatment programmes from both numerator and denominator rather than counting them as exits.

Outcome% of infants aged 0-5 months exclusively breastfed in the previous 24 hoursNutritionPercentage
- Definition & calculation
Numerator: Number of surveyed infants aged 0-5 months who received only breast milk (no other food or liquids, including water) during the previous day
Denominator: Total number of infants aged 0-5 months surveyed
Using 24-hour maternal/caregiver recall, calculate the proportion of infants under 6 months fed only breast milk, following WHO/UNICEF IYCF indicator definitions.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Household or KAP survey using the standard WHO/UNICEF IYCF recall module
- Guidance & common pitfalls
- Based on a single 24-hour recall, this estimates population-level practice, not each infant's full feeding history; ensure the sample adequately captures the narrow 0-5 month band, which often yields small sub-samples with wide confidence intervals.

Output% of children aged 6-59 months who received two doses of vitamin A supplementation in the past 12 monthsNutritionPercentage
- Definition & calculation
Numerator: Number of children aged 6-59 months who received two doses of vitamin A supplementation at least four months apart during the past 12 months
Denominator: Total estimated number of children aged 6-59 months in the target population/catchment for the same period
Divide the number of children 6-59 months recorded as receiving two vitamin A doses in the past year by the estimated target population of that age group and multiply by 100, validating administrative tallies against a coverage survey where feasible.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Supplementation campaign tally sheets and registers, health/nutrition facility records, and post-event coverage survey
- Guidance & common pitfalls
- Distinguish administrative (tally-based) coverage, which is prone to denominator error, from survey-measured coverage; count only children who reached the full two-dose annual schedule, as a single dose does not meet it. Outcome% of assisted households able to maintain a safe indoor temperature inside their shelter throughout the cold seasonShelter & NFIPercentage
- Definition & calculation
Numerator: Number of surveyed cold-climate assisted households whose shelter maintains a safe indoor temperature throughout the cold season (self-reported adequacy and/or measured against an agreed threshold)
Denominator: Total number of assisted households in cold-climate areas surveyed
Divide the number of surveyed cold-climate households whose shelter maintains a safe indoor temperature during winter (via reported adequacy and/or measured readings against an agreed threshold) by all surveyed cold-climate households, multiplied by 100.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Winterization outcome monitoring survey, with optional spot temperature measurement during shelter visits
- Guidance & common pitfalls
- Use in cold-climate and winterization responses; captures thermal performance (insulation, sealing, heating) as a narrower construct distinct from general protection from the weather and elements. Account for safe heating practices, since unsafe heating creates carbon-monoxide and fire risks. Define the temperature threshold in advance where measurement is used.

Output% of supported water points functional at time of assessmentWASHPercentage
- Definition & calculation
Numerator: Number of supported water points found fully functional and delivering water at time of assessment
Denominator: Total number of supported water points assessed
Physically verify a sample of supported water points and calculate the proportion delivering water to standard at the time of assessment.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- Water point functionality survey, spot checks, operator/committee records
- Guidance & common pitfalls
- A sustainability/service-continuity proxy at output level; distinguish fully functional from partially functional and record downtime and repair status to interpret trends.

OutputNumber of hygiene kits meeting agreed standards distributed to targeted householdsWASHNumber
- Definition & calculation
Count the number of standard-compliant hygiene kits distributed to targeted households as recorded in distribution and logistics documentation.
- Means of verification
- Distribution records, waybills, post-distribution monitoring
- Guidance & common pitfalls
- An output of items delivered, not people reached or items used; specify kit composition and pair with PDM to confirm receipt and adequacy.

OutputNumber of people provided with access to a safe water source through the interventionWASHNumber
- Definition & calculation
Sum the number of individuals served by newly constructed, rehabilitated or supported water points, applying an agreed maximum users-per-source ratio to avoid over-counting.
- Means of verification
- Distribution and construction records, beneficiary registration, water point inventories
- Guidance & common pitfalls
- Count people served, not water points; deduplicate across multiple sources and reconcile design capacity against actual users to prevent inflation.

OutputNumber of people reached with hygiene promotion messages and activitiesWASHNumber
- Definition & calculation
Count individuals reached through hygiene promotion sessions, household visits and campaigns, deduplicating repeat participants where records allow.
- Means of verification
- Activity registers, session attendance sheets, promoter reporting
- Guidance & common pitfalls
- Reach is an output, not a behaviour-change result; avoid double-counting the same people across multiple sessions and pair with a behaviour outcome indicator.

OutputNumber of people with access to functioning, sex-segregated and lockable sanitation facilitiesWASHNumber
- Definition & calculation
Count the number of people served by functioning latrines that are sex-segregated, lockable and accessible, applying the agreed users-per-latrine planning ratio.
- Means of verification
- Facility inventory, construction records, latrine mapping
- Guidance & common pitfalls
- Count only facilities that are functional and safely usable; separation, lighting and lockability are part of the standard, not optional extras.

OutputRatio of users per functioning sanitation facility (latrine)WASHRatio
- Definition & calculation
Divide the population served in a defined location by the number of functioning latrines to derive average users per facility.
- Means of verification
- Facility inventory, headcount/population data, latrine functionality checks
- Guidance & common pitfalls
- Count only functioning latrines; report against the context-agreed target (e.g. shared facility ratios in camp vs urban settings) rather than a universal number.

Activity% of planned water-quality tests completed within the scheduled monitoring periodWASHPercentage
- Definition & calculation
Numerator: Number of scheduled water-quality tests (e.g. free residual chlorine, turbidity, E. coli) that were carried out on or before their planned date during the reporting period
Denominator: Total number of water-quality tests planned in the workplan for the reporting period
Divide the number of planned water-quality tests actually completed by their scheduled date by the total number of tests planned in the period, then multiply by 100.
Formula: (Numerator ÷ Denominator) × 100
- Means of verification
- WASH monitoring workplan, water-quality testing logs and field monitoring checklists
- Guidance & common pitfalls
- Measures adherence to the monitoring schedule, not water safety itself; a test counts only if completed on or before the planned date. Use to track whether routine surveillance is being delivered on time, and pair with a separate result on the share of samples meeting quality standards.
