Numerator: Number of identified service gaps referred to responsible actors and resolved or actioned within the agreed timeframe
Denominator: Total number of service gaps identified through site monitoring during the reporting period
Gaps logged during site monitoring are tracked through referral to resolution; those closed or actioned within the agreed timeframe are divided by all gaps identified.
Formula: (Numerator ÷ Denominator) × 100

Measures the effectiveness of CCCM's gap-identification and advocacy role; requires a functioning referral and tracking system and clearly defined timeframes. Note that resolution often depends on other actors beyond the reporting agency's control.
