Definition & calculation
Numerator: Number of supported health facilities recording zero stock-out days for every item on the pre-agreed tracer/essential-medicine list during the reporting period
Denominator: Total number of supported health facilities assessed during the reporting period
For each facility, check stock records against the pre-agreed tracer medicine list; a facility qualifies only if none of the tracer items were out of stock on any day in the period.
Formula: (Numerator ÷ Denominator) × 100

Define the tracer list up front (e.g. amoxicillin, ORS/zinc, artemisinin-based combinations, oxytocin) and apply it consistently across facilities and periods. Measures supply-chain reliability, not appropriateness of consumption or prescribing.
