Indicator

% of planned supportive supervision / mentoring visits to supported health facilities completed

Definition & calculation

Numerator: Number of supportive supervision or clinical mentoring visits actually conducted (with a documented report) during the reporting period

Denominator: Number of supportive supervision or mentoring visits planned/scheduled in the work plan for the reporting period

Divide completed visits (with a documented report) by the number of visits scheduled in the work plan for the period.

Formula: (Numerator ÷ Denominator) × 100

Means of verification

Supervision visit reports; activity work plan; supervision checklists

Interpretation

Direction: Higher is better

Typical reporting frequency: Per activity cycle / continuous

Guidance & common pitfalls

A process/completion measure of implementation, not of service quality. Count only visits with documented findings, not brief or undocumented drop-ins.

This library is a curated reference to adapt, aligned generically with recognised humanitarian frameworks (Sphere, cluster registries, IASC, CHS and others). It is not an official indicator registry — always use the exact indicator definitions your donor or cluster requires.
